Here is a zip archive of a template for taking detailed notes about a source (book, PDF, video, presentation, etc.).
Expand it and add add the OmniOutliner file to your custom templates folder. Rename it and edit it to suit your needs. You can even replicate it to create other templates, for instance for taking notes about stories (films, plays, etc.)
